This All About Worksheet will help you get to know your middle school and high school students! This back to school activity is quick and easy, and gives your students an opportunity to be creative while providing “all about me” information.

What’s Included in this All About Me Worksheet:

• Three Versions with Student Instructions:
  • Simple Tree to find out the basics, like their favorite color, animal, band, food, and store which are written on the roots. The multiple leaves are used to record students’ strengths, challenges, must have apps, favorite survival tools and hobbies.
  • Tree with Roots and Clouds includes all the student information recorded on the “Simple Tree” plus clouds and rocks. On the clouds, the students record their five dreams. The rocks represent their five hurdles.
  • Add the Rainbow includes all the student information recorded on the “Simple Tree” and the “Tree with Roots and Clouds” plus a rainbow. On the rainbow, the students list five adjectives to describe themselves.

Instructions for All About Worksheet:

1. Choose which version your students will complete
2. Print out the trees
3. Students write down their student information
4. Students color and decorate using markers, colored pencils, or gel pens.
5. Collect and/or display the trees.
